Objectives – Participants will be able to:

  1. Recognize that completion of advanced directives is part of respect for patient goals of care
  2. Review ongoing efforts to improve advanced directive completion rates in the Norris Cotton Cancer clinics and the inpatient wards of DHMC
  3. Discuss approaches to improving advanced directive completion at DHMC and beyond

 

Tim Lahey, MD MMSc, is Associate Professor of Medicine and of Microbiology & Immunology at the Geisel School of Medicine at Dartmouth.  A practicing HIV physician and HIV/TB researcher, Dr. Lahey is chair elect of the DHMC Bioethics Committee and head of the DHMC Inpatient Advanced Directives Initiative.

Ira Byock, MD is Professor of Medicine & Community and Family Medicine at the Geisel School of Medicine at Dartmouth, and the Director of Palliative Medicine.  He is a Past President (1997) of the American Academy of Hospice and Palliative Medicine.  He has served as Director for Promoting Excellence in End-of-Life Care, a national grant program of the Robert Wood Johnson Foundation.  Dr. Byock has authored numerous articles on the ethics and practice of hospice, palliative and end-of-life care.

Claire Pace, APRN, MSN is an Instructor in Medicine at the Geisel School of Medicine at Dartmouth.  She is a nurse practitioner with Dartmouth Hitchcock Medical Center in the Radiation Oncology Department.  She has worked with patients who have a diagnosis of cancer for nearly forty years--from the time of diagnosis through end of life.  Her roles have included direct patient care in home care and hospice, as well as leadership roles which included program and service development for cancer patients in CT and MA communities.
